1. Current Pending Formal Applications for products listed above:
- Switch to Wealth Builder IUL: A new supplemental application (form #ICC18-ZM- 2591 0EU or applicable state variation) and a revised signed illustration will be required prior to issue.
- Remain with existing product: All delivery and premium requirements must be received by the administrative office in good order and the policy must be placed inforce by December 28, 2019.
2. New Formal Applications for products listed above:
All formal applications must be received by the administrative office in good order by October 14, 2019. All delivery and premium requirements must be received by the administrative office in good order and the policy must be placed inforce by December 28, 2019.
3. Requests to Reissue a recently placed case using products listed above:
